Each participant must also complete an online Participant Health Information Form. A link to this form will also be included in the registration confirmation email.
Minors (under the age of eighteen) must also have a Medical Release/Permission for Medical Treatment Form signed by parent or legal guardian. The School Director will send this form via email to the parent/guardian to complete and return in advance of the event.
In the event of a medical problem, many locations on the Mulberry River are remote and cannot be easily accessed by emergency medical services. This lack of quick access to help is one of the risks we all must understand and accept in being outdoors and especially when paddling whitewater rivers like the Mulberry.
Some things that you can do to lessen these risks are:
- Prepare for your needs. Obtain adequate quantities of your medications and have appropriate means to carry them with you on the river. Talk with your doctor and explain that in this school, you will be in remote locations and may be hours away from medical care.
- Inform your Instructor of any medical condition such as: allergies, asthma, epilepsy, diabetes, infections, medications, recent injury or surgery, and significant physical limitations. If you carry emergency medications or injections, tell your Instructor about how and where you carry them, and bring an extra supply in a separate container so that if one is lost, there is another dose available. Please indicate on the registration form and/or Participant Health Form if you have any of these medical issues that should be discussed with your Instructor.
